Frames		Is Dr. Look accepting new patients? Yes. Dr. Look is currently accepting new patients. Why should I prefer to see a licensed psychologist? Dr. Look is a licensed psychologist. As a licensed psychologist, Dr. Look has achieved the training, experience and examination requirements to practice psychology as set forth by the State of Washington. Her license (#1653) was issued in 1993. She is a member of the American and Washington State Psychological Associations. Are Dr. Look's services covered by my health insurance? Dr. Look currently accepts payments directly from these Insurers: First Choice Health Network Premera Blue Cross Regence Blue Shield Aetna Cigna ValueOptions Uniform Medical Plan (WA state) Tricare Crime Victims' Compensation Labor and Industries (WA Worker's Comp) Dr. Look is frequently being accredited by new insurance companies. Please call if your health insurance is not listed above. Because each of these insurance companies provide many different insurance policies, it is your responsibility to determine if your particular policy covers psychological services, requires referral and/or pre-approval for services. Can I still see Dr. Look if my health insurance is not listed above? Yes. Dr. Look will provide a statement of services to you that you can use to file a claim with your insurance company. You will be responsible for paying Dr. Look the full amount for services provided. What if my health insurance has a copay? Dr. Look prefers payment of copays at the time of your appointment. What if my insurance does not cover psychological services? You can see Dr. Look but you will be responsible for paying for each visit. What if I don't have health insurance? You can see Dr. Look but you will be responsible for paying for each visit. Can I pay for my visit with a credit card? Dr. Look accepts *Visa* and *MasterCard* credit cards. How long is an appointment? Your first visit will be 75 minutes. Subsequent visits are usually 50 minutes. What happens if I am late for my appointment? You can see Dr. Look for the remainder of your appointment. What happens if I cannot keep my appointment? You must call the office to cancel your appointment at least 24 hours before your scheduled visit. If you fail to notify Dr. Look at least 24 hours before your appointment, you will be responsible for paying for the missed appointment. What if I need to see Dr. Look in the evening or during the weekend? Evening and weekend appointments are available by special arrangement. Call Dr. Look's office. What if I have an emergency? Call Dr. Look's office and her answering service will attempt to reach her or her backup on call psychologist. Tell the message taker that your message is urgent. If Dr. Look or her backup on call psychologist cannot be reached, call the Crisis Clinic (206) 461 3222, call 911 or go to your local emergency room. How confidential is the information I share with Dr. Look? Your medical and billing information is held confidential by Dr. Look in strict accordance with her "Disclosure of Psychology Practice" policies. This document lists the exceptions to confidentiality required by various laws. You will receive a copy of this policy during your first visit with Dr. Look.
